Daisy Ortiz, 75 of Madison, passed away on Friday December 14, 2018 at the Tri Star Skyline Medical Center in Nashville, TN. Daisy was born on October 23, 1943 to parents the late Rafeal and Cristina Ortiz in Yauco, Puerto Rico. She is survived by her husband Nestor Irizarry, a son and daughter, several brothers, sisters, nieces and nephews. Visitation for Daisy with friends and family was at the Boyd Funeral Home in Ashland City on Monday December 17th and her remains were later flown to San Juan, PR to be buried with family.
